Welcome to Home Heart Health Inc. ("we," "us," or "our"). These Terms and Conditions ("Terms") govern the provision of home nursing services ("Services") to you or the individual receiving care ("Client"). By engaging our Services, you agree to be bound by these Terms. Please read them carefully.

1. Scope of Services
   - We provide professional home nursing services, including but not limited to skilled nursing, personal care, medication management, and health monitoring, as agreed upon in the Client’s individualized care plan.
   - Services are delivered by licensed, certified, or trained personnel in accordance with applicable federal, state, and local regulations.
   - The scope of Services will be outlined in a written agreement or care plan, which may be updated as needed with mutual consent.

2. Client Responsibilities
   - Clients or their authorized representatives must provide accurate and complete health information, including medical history, allergies, and current medications, to ensure safe and effective care.
   - Clients must maintain a safe and accessible home environment for our staff, free from hazards, harassment, or unsafe conditions.
   - Clients are responsible for promptly notifying us of any changes in health status, care needs, or concerns about the Services provided.

3. Payment Terms
   - Fees for Services will be outlined in the service agreement. Payment is due as per the agreed schedule (e.g., weekly, monthly) unless otherwise specified.
   - We accept payments via [insert accepted payment methods, e.g., credit card, bank transfer, insurance]. Clients are responsible for verifying insurance coverage, if applicable.

4. Cancellations and Scheduling
   - Clients must provide at least 48 hours notice for cancellation of scheduled Services to avoid a cancellation fee.
   - We reserve the right to reschedule or cancel Services due to unforeseen circumstances (e.g., staff illness, emergencies) and will make reasonable efforts to provide timely notice and arrange alternative care.

5. Privacy and Confidentiality
   - We comply with Provincial and Federal Health regulation policies and other applicable privacy laws to protect Client information.
   - Client health information will only be shared with authorized personnel or third parties (e.g., physicians, insurers) as necessary for care or as required by law.
   - Clients may request a copy of our Privacy Policy for additional details.

6. Liability and Indemnification
   - We are not liable for injuries or damages arising from Client negligence, failure to follow care instructions, or unsafe home conditions.
   - Clients agree to indemnify and hold us harmless from claims arising from inaccurate information provided by the Client or unauthorized actions by third parties in the Client’s home.

7. Termination of Services
   - Either party may terminate Services at any time, without notice.
   - We may terminate Services immediately for non-payment, unsafe conditions, or behavior that endangers our staff, with written notice to the Client.
   - Upon termination, Clients remain responsible for payment of Services rendered up to the termination date.

8. Amendments
    - We reserve the right to update these Terms at any time. Clients will be notified of material changes in writing or via email, and continued use of Services constitutes acceptance of the revised Terms.
